HOW TO SUBMIT AN APPLICATION
ONLINE: (Please read
instructions carefully):
This form is in PDF format. PDF
format allows you to view your
form electronically on most
computers. The freely available
Adobe Acrobat Reader is required
to view and print PDF files,
available for
free download.
The Employment Application form
may be completed online by
filling in the blank fields,
selecting the corresponding
boxes, and pressing the “submit”
button once at the bottom of the
application form. Once the
submit button is pressed, you
will see a screen that shows
options for your completed
application. You may, at this
point, print a copy of your
completed application but you
may not save it to your
computer. Also, please note
that you will not
receive an electronic
confirmation of submission. It
is the responsibility of the
applicant to confirm that the
submission of the application
was successful and that it was
received by the closing date.
Some positions require the
completion of a supplemental
questionnaire and/or resume’. If
the position you are applying
for requires these documents,
you must submit them separately
in Microsoft Word or text format
to
Helpdesk@nwos-spokane.com.
Application materials submitted
without these documents will be
rejected as incomplete. Other
required documents, such as
typing certificates, DD214,
etc., may be mailed to our
mailing address listed below.
In order to submit your online
application form to us for
consideration, we need to
transmit your personal details
over the internet. Although this
site utilizes Secure 128-bit
Encryption technology (SSL), the
security of your data is not
guaranteed by Northwest
Orthopaedic Specialist, P.S..
If you are concerned about the
sensitivity of any of the
personal information contained
in your application form you
should print a copy of the
application and mail, fax or
drop off your application at our
main office (address and fax
number details can be found
listed below).
